Important Health Coverage Tax Documents

The 2025 1095 tax documents are prepared and available upon request. Form 1095s provide information about offers of coverage made to full-time employees. Partners' 2025 benefit eligible employees can access these at any time in Paycom under Payroll > Year-End Tax Forms, and then click on 1095. The form can be viewed, downloaded, or printed within Paycom.

Partners is no longer required to mail hard copies to current or previous employees due to the Paperwork Burden Reduction Act, IRS Notice 2025-15. If you require a physical copy be mailed to you, please contact the Benefits Department at:
