Call the Partners Intake Coordinator at 952-405-2582 and they will help you to determine if an assessment is appropriate and work with you to fill out the necessary paperwork. (You can download the intake forms by clicking on “download forms” in the location.) We will also help you to obtain the necessary referral from your child’s physician.
The Assessment:
The assessment that is administered is a standardized, observational clinical assessment. Parent reports are used by the therapists to assess your child’s current level of functioning. Evaluations typically fall between 60-90 minutes in length depending on the type and number of tests that are completed.
Results and recommendations will be discussed immediately following the evaluation and a full written report will be mailed to the family within 2 weeks of the evaluation.
