Call the Partners Intake Coordinator at 952-405-2582 and they will help you to determine if an assessment is appropriate and work with you to fill out the necessary paperwork. (You can download the intake forms by clicking on “download forms” in the location.) We will also help you to obtain the necessary referral from your child’s physician.
You will also need to contact your insurance company. Download and complete a Health Insurance Benefits Worksheet while talking with your insurance company to see if occupational therapy services are covered by your plan.
The Assessment:
The assessment that is administered is a standardized, observational clinical assessment. Parent reports are used by the therapists to assess your child’s current level of functioning. Evaluations typically fall between 60-90 minutes in length depending on the type and number of tests that are completed.
Results and recommendations will be discussed following the evaluation that includes a full written report.
