Enrollment Procedure
Contact us online – Fill out our Contact Form online and our Enrollment Coordinator will contact you to guide you with your next steps.

Contact us by phone – (952) 818-2876 for Minnesota services or (608) 785-4111 for Wisconsin services. Our Enrollment Coordinator is available from 8:00 a.m. to 5:00 p.m., Monday – Friday to assist you. You should expect to be on the phone for approximately 30 minutes. Please be ready to provide the following information:
- Your contact information
- How did you find out about Partners In Excellence
- Information about your child
- Insurance information – Provider(s) and policy number(s)
- Brief medical history and information related to your child’s Autism Spectrum Disorder (ASD) diagnosis
- School evaluations and testing (if applicable)
Next steps
After you have spoken with our Enrollment Coordinator, the next step would be to gather medical and educational documents and fill out Partners’ Enrollment Packet.
In addition to the Enrollment forms, please provide any/all of the following:
- Psychological and/or diagnosis evaluations and reports
- Occupational, Speech and Physical Therapy reports and treatment plans
- Most recent IEP and 3 year re-evaluation
- Medical reports
- Copy of front and back of all insurance cards (required)
